Refund and Cancellation Policy

At Suraji Health & Wellness, we are dedicated to providing individualized, high-quality clinical care. Due to federal and state regulations governing healthcare providers, diagnostic testing, prescription medications, and administrative scheduling, all sales, subscriptions, and services are subject to the following policies.
1. Prescription Medications & Products (GLP-1 & Peptide Therapies)
Due to medical safety regulations, state pharmacy laws, and cold-chain storage requirements, all prescription medications—including GLP-1 therapies and peptide protocols—are strictly non-returnable and non-refundable once ordered or dispensed.
- Pharmacy Processing: Once your prescription order has been submitted to or fulfilled by our partnering compounding pharmacy, the order cannot be canceled, paused, or refunded.
- Returns & Exchanges: Federal law prohibits the re-dispensing or re-salvage of prescription medications once they have shipped. We cannot accept returned medications under any circumstances.
- Damaged, Defective, or Incorrect Shipments:
If your shipment arrives damaged, temperature-compromised, broken, or with missing items, you must contact us at [email protected] or [email protected] within 48 hours of delivery.
Please include photos of the damaged packaging, shipping label, and contents. Verified damaged or incorrect orders will be replaced or re-dispensed through the pharmacy at no additional cost.
2. Multi-Month Treatment Plans & Payment Schedules
For multi-month weight management or peptide treatment packages, Suraji Health & Wellness offers flexible recurring payment plans to spread program costs over the course of care. By enrolling in a multi-month payment plan, you agree to the following financial terms:
- Automatic Recurring Billing: Payments are automatically charged to your credit or debit card on file on your scheduled monthly billing date.
- Late / Failed Payment Fee: If an automated recurring payment declines or fails to process, a $35 failed payment fee will be assessed to your account following a 3-day grace period.
- Treatment & Shipping Pauses: If an account balance remains past-due, all active prescription refills, compounding orders, and medication shipments will be placed on immediate administrative hold. Services and shipments will only resume once the past-due balance and late fees are paid in full.
- Early Termination: Multi-month packages offer discounted pricing based on a commitment to a complete protocol. Early cancellation forfeits package discounting, and any unpaid balances for clinical services or medications already ordered remain the patient's financial responsibility.
3. Clinical Consultations & Professional Services
Our clinical fees cover the time, expertise, review of medical history, and personalized care planning provided by our licensed healthcare providers.
- Completed Consultations: All completed clinical appointments—including Initial Menopause Consultations, Lab Review Sessions, and Follow-Up Visits—are 100% non-refundable. Once clinical time has been delivered, services cannot be unrendered.
- Appointment Cancellations & Rescheduling:
Requests to cancel or reschedule must be submitted at least 24 hours prior to your scheduled appointment time.
Late cancellations (under 24 hours) or missed appointments ("no-shows") may result in a forfeiture of the consultation fee or a $50 late fee.
4. Diagnostic Testing & Ordered Lab Work
- Submitted / Drawn Labs: Once a lab requisition has been generated and submitted to the diagnostic facility (e.g., Quest Diagnostics), or after a blood draw/sample collection has taken place, lab fees are strictly non-refundable.
- Unused Lab Requisitions: If you purchase a lab panel and request a cancellation before the lab order is processed or performed, you may receive a refund minus a $25 administrative/merchant processing fee.
- Lab Expiration: Lab requisitions must be completed within 90 days of issuance unless otherwise directed by your clinician. Expired lab orders are non-refundable.
5. Policy Summary Matrix
GLP-1 & Peptide Products: Non-Refundable
Pharmacy regulations prohibit returns. Replacements offered for damaged goods reported within 48 hours.
Multi-Month Payment Plans: Non-Refundable
Failed payments incur a $35 late fee; refills/shipments paused until resolved.
Completed Consultations: Non-Refundable
Covers provider time, chart review, and treatment planning once rendered.
Ordered / Drawn Labs: Non-Refundable
Non-refundable once requisition is submitted to lab or sample collected.
Unprocessed Lab Orders: Partial Refund
Eligible for refund minus administrative processing fee prior to processing.
Late Cancellation / No-Show: Non-Refundable
Cancellations require 24+ hours advance notice.
